Needed to try out for any sport:
1. Participation form - available for pick-up or here
2. Sports physical from student's doctor (can be any time - physical expires after 365 days)
3. Athletic insurance: https://www.hsri.com/K12_Enrollment/Main/default.asp
All 3 steps must be completed before any student-athlete can participate in a tryout and/or practice.
